The Barclays Premiership side had gone behind to an early goal from Grzegorz Rasiak before Papa Bouba Diop levelled from the penalty spot.
Luis Boa Morte put Fulham ahead before substitute Paul Peschisolido took the game into extra-time when John and Jensen struck.
Derby were unfortunate not to beat their opponents at the first attempt and they got the perfect start in the replay with a wonderfully worked goal within the first two minutes.
It stemmed from a free-kick on the right side by Inigo Idiakez as he sent Tommy Smith racing down the right wing and the former Watford man pulled back for Poland striker Grzegorz Rasiak to poke in from close range.
Morten Bisgaard made good progress before sliding the ball back for Adam Bolder but his shot from 18 yards was well struck but too close to Edwin van der Sar and the Dutchman saved comfortably.
Bisgaard fired in a long-range effort which van der Sar fumbled but then gathered before Rasiak could pounce.
Smith sped down the left and his dangerous cross was headed against his own post by Zesh Rehman before van der Sar gratefully cleared.
Chris Coleman's men were struggling and the nearest they came to scoring in the opening exchanges was when Boa Morte's free-kick got caught in the wind but dropped over Lee Camp's head.
Fulham looked doomed to finish the half in arrears to their Coca-Cola Championship opponents but the home side were gifted the chance of an equaliser on the stroke of half-time.
Defender Michael Johnson handled in the area and Diop converted the penalty.
Fulham made a change at the break with the out-of-sorts Rehman being replaced by French defender Alain Goma.
The home side were ahead within five minutes of the restart as a swirling cross from Tomasz Radzinski dropped to Boa Morte at the back post.
The Portuguese international neatly brought the ball down and cut inside before firing a precise low shot through a crowd of players and past Camp from 12 yards.
Derby substitute Junior sent Bisgaard racing clear on goal but the Dane never looked convincing and his low shot was too near van der Sar, allowing the Dutchman to save.
Idiakez lined up a free-kick more than 30 yards from goal but his curling effort still had Fulham's goalkeeper scurrying across his goal to gather.
The Spanish star tried his luck with another long-range effort soon afterwards but this time cleared the bar.
Fulham could have made the game safe on the hour as Lee Clark's cross found Boa Morte six yards out but his header was too near Camp who managed to gather at the second attempt.
And on their very next attack Radzinski pulled back for Clark who drilled a low cross shot from the edge of the box narrowly wide of the post.

Zat Knight - who was looking more composed with Goma at his side - sent a header straight at Camp after a clever free-kick by Clark.
Junior crossed for Marcus Tudgay who headed goalwards but van der Sar was again on hand to save.
Clark was having more influence on the match - showing no effects of the calf problem which kept him sidelined for over a year - and he went close again with a long-range effort 20 minutes from time.
Idiakez blazed a volley wildly off target after Goma had failed to clear his lines for the home side.
Liam Rosenior's excellent sweeping pass put Radzinski in on goal but Camp dashed out to save at the feet of the Canada international and keep his side's hopes alive.
Derby boss George Burley threw on former Fulham striker Peschisolido in the closing stages in a desperate bid to take the tie into extra-time.
And it had the desired effect four minutes from time as another Idiakez free-kick was clawed away by van der Sar but the Canadian forward poked in the equaliser from close range.
Substitute Collins John put Fulham ahead with a low shot from close range in extra-time.
And Jensen, also on as a sub, put the game beyond the visitors with a finish from 18 yards.
